Gayet–Wernicke encephalopathy: a complication not to be overlooked in patients with catatonic schizophrenia

Abstract Background Gayet–Wernicke's encephalopathy (GWE) is a life-threatening neurological emergency caused by vitamin B1 deficiency. This rare complication, which may be reversible if managed promptly. However, any diagnostic or therapeutic delay exposes to the risk of serious sequelae. Although this pathology frequently occurs in chronic alcohol users, it can also occur situation severe undernutrition. In context, we report case GWE occurring after undernutrition complicating prolonged catatonia patient suffering from schizophrenia. Case presentation He was 47-year-old patient, esophageal stenosis, and followed psychiatry for schizophrenia, who hospitalized catatonic relapse. put on benzodiazepine with parenteral rehydration. Faced persistence food refusal, appearance signs undernutrition, diet introduced. The evolution marked deterioration his general condition, mental confusion. examination showed central vestibular syndrome, brain magnetic resonance imaging hyperintense signaling periaqueductal area, mammillary bodies, medial thalamic nuclei T2-FLAIR sequences. suspected parental therapy initiated. Laboratory testing confirmed diagnosis, showing low thiamine blood level 32 nmol/l [normal range: 78 185 nmol/l]. Regression symptoms observed within 2 weeks treatment. Conclusion Through illustration, draw attention psychiatrist, must think about preventing supplementing at developing B1.
